What is NAD+?

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every living cell. It cycles between two forms, NAD+ and NADH, and is required for the reactions that convert food into cellular energy. It also serves as a substrate for enzyme families such as sirtuins and PARPs that take part in cellular repair and signaling.

Compounded NAD+ Injection is a sterile solution of NAD+ prepared for injection. At East Coast Compounding & Pharmacy, we compound it at 200 mg/mL in a 10 mL multiple-dose vial, with the dose and schedule set by your prescriber.

Research has reported that NAD+ levels in several tissues decline with age, which is one reason providers include NAD+ in wellness and longevity plans. Whether NAD+ is appropriate for you is a decision for your prescriber.

*East Coast Compounding is a 503A pharmacy, which means all patients must submit a prescription from a legal prescriber to order this product. Medication facts on this page are for informational purposes and come from the sources listed in our FAQ.

How Do NAD+ Injections Work?

NAD+ is the same molecule your cells already make and use. In the body it accepts and donates electrons during energy metabolism, and it is consumed by enzymes that repair DNA and regulate cell signaling. Because it is used up in those reactions, cells continuously rebuild their NAD+ supply from precursors in the diet.

An injection delivers NAD+ into the tissue under the skin or into muscle, where it is absorbed into the bloodstream without passing through the digestive system. Most published human research on raising NAD+ has used oral precursors such as nicotinamide riboside; a small pilot study has measured how NAD+ given intravenously appears in blood and urine over time.

The compounded injection is prepared from NAD+ as the active ingredient in a sterile aqueous base. Your prescriber chooses the dose, how often it is given, and whether it is given subcutaneously or intramuscularly.

Product Specifications

NAD+ Injection is given as a shot, most often under the skin (subcutaneous) or into a muscle (intramuscular). The dose, how often you inject, and how long you continue are decided by your prescriber based on your treatment plan and how you respond.

Prescribers commonly start at a lower dose and adjust it over the first weeks based on tolerance. If you miss a dose, follow the instructions on your prescription label or contact your prescriber; do not double a dose to catch up.

At East Coast Compounding & Pharmacy, we compound NAD+ Injection at 200 mg/mL in a 10 mL multiple-dose vial, prepared to the instructions on the prescription. Each vial is labeled with its strength, beyond-use date, and storage instructions.

The most commonly reported effects with NAD+ injections are at the injection site, including pain, redness, or a small lump that resolves on its own. Some patients report nausea, flushing, headache, or fatigue around the time of the injection.

With intravenous NAD+ infusions, chest or abdominal tightness, cramping, and nausea have been reported during the infusion and were related to how quickly it was given; slowing the rate reduced them. Injecting slowly and following your prescriber's technique instructions is recommended for the same reason.

Contact your prescriber if you notice persistent symptoms, a severe reaction at the injection site, or any signs of an allergic reaction such as rash, swelling, or trouble breathing. Our pharmacists are also available to answer medication-related questions.

Do not use NAD+ Injection if you have had an allergic reaction to NAD+ or to any ingredient in the compounded solution. Tell your prescriber about any history of allergies to injectable medications.

The safety of NAD+ injections during pregnancy or breastfeeding has not been established. Patients who are pregnant, planning a pregnancy, or nursing should discuss this with their provider before use. This product is not intended for children unless specifically prescribed.

Tell your prescriber about all medical conditions, including heart, liver, or kidney conditions, before starting. NAD+ Injection is compounded for an individual patient and should not be shared.

Published data on drug interactions with injected NAD+ are limited. Give your prescriber and pharmacist a complete list of prescription medications, over-the-counter products, and supplements you take, including other NAD+ precursors such as nicotinamide riboside or NMN.

Do not mix NAD+ Injection with other injectable medications in the same syringe unless your prescriber has specifically directed it. Ask our pharmacists if you are unsure how to schedule NAD+ around other injections.

Store NAD+ Injection in the refrigerator at 36 F to 46 F (2 C to 8 C). Keep the vial in its carton or otherwise protected from light, because NAD+ solutions are light-sensitive. Do not freeze.

Before each dose, look at the solution. It should be clear and colorless to pale yellow with no particles. Do not use it if it is cloudy, discolored, or contains particles, or after the beyond-use date printed on the label. Wipe the vial top with alcohol before each draw, and discard the vial according to your label once it is opened past its beyond-use period.

How to Administer an NAD+ Injection

NAD+ Injection is typically given as a subcutaneous injection into the fatty tissue of the abdomen or outer thigh, or as an intramuscular injection into a large muscle such as the outer thigh or upper outer buttock. Your prescriber will tell you which method and which needle to use.

Let the vial come to room temperature for a few minutes after taking it out of the refrigerator. Wash your hands, clean the vial top with an alcohol swab, and draw up the exact volume on your prescription label.

Clean the injection site with an alcohol swab and let it dry. For a subcutaneous injection, pinch a fold of skin and insert the needle at a 45- to 90-degree angle; for an intramuscular injection, insert at 90 degrees. Push the plunger slowly and steadily. A slower injection is more comfortable and is the approach prescribers recommend for NAD+.

Rotate injection sites with each dose to prevent irritation, and place used needles in a sharps container. Return the vial to the refrigerator right away.

Consult your medical provider or nurse to learn the proper technique for administering a subcutaneous or intramuscular injection safely. Have additional questions? Contact our compounding pharmacy.
