What is Testosterone Cypionate?

Compounded Testosterone Cypionate is an injectable form of testosterone.

Testosterone is a sex hormone known for maintaining muscle mass, bone density, energy, mood, and libido. As testosterone declines, men may begin to feel tired, experience trouble sleeping, shifts in mood, loss of muscle, or difficulty with intimacy.

Testosterone Cypionate is commonly prescribed as a replacement therapy for men diagnosed with low testosterone (Low T) or Andropause. Since it is compounded in an oil base, the testosterone is released slowly into the bloodstream. This extends the duration of the medication's effects over several days.

*East Coast Compounding is a 503A pharmacy, which means all patients must submit a prescription from a legal prescriber to order this product.

How Do Testosterone Cypionate Injections Work?

Testosterone Cypionate injections are a type of synthetic Testosterone.

While the active hormone itself is identical to the testosterone produced naturally in the body, the cypionate ester attached to it is created chemically in a lab to slow down the hormone's release and extend its duration of action after injection.

The main purpose of the cypionate is to make the testosterone more lipid-soluble. This makes it dissolvable in oil, allowing it to be administered via intramuscular injection in an oil base, such as sesame oil.

The cypionate chemical modification slows down the rate at which the testosterone is released and absorbed from the injection site into the bloodstream. This means injections do not need to be given daily.

Product Specifications

Testosterone Cypionate is given by a shot into a muscle, typically in the thigh or buttocks. The dose and how often you get the injection are decided by your doctor based on your personal hormone levels and how you respond to the injection.

Since the cypionate chemical slows the release of the hormone, shots are typically given every one to four weeks. If you miss a shot, inform your doctor.

At East Coast Compounding & Pharmacy, we custom-tailor our compounded Testosterone Cypionate injections to fulfill prescriber instructions. We offer the following strengths and vial sizes: Testosterone Cypionate (Compounded in Sesame Oil) 20 mg/mL: available in 3 mL, 4 mL, and 5 mL vials. Testosterone Cypionate (Compounded in Sesame Oil) 200 mg/mL: available in 5 mL, 8 mL, and 10 mL vials. Testosterone Cypionate (Commercial)† 200 mg/mL: available in 1 mL and 10 mL vials.

The most common side effects from Testosterone Cypionate may involve dermatologic issues, such as acne, oilier skin, and increased density and distribution of body hair. Since this injection replaces the body's natural production of testosterone, patients might also notice testicular atrophy or decreased sperm count.

Other frequent effects include pain or swelling at the injection spot, water retention, and changes in mood. Less common but more serious risks include issues with your liver, changes in cholesterol, and a higher chance of developing blood clots.

Clinicians and patients must remain vigilant for less common, yet serious, side effects. These include changes in cholesterol and lipid profiles, as well as an elevated risk of developing blood clots.

Long-term monitoring is required to assess potential risks to the prostate and liver. Patients experiencing persistent or severe symptoms should consult their healthcare provider immediately for dose adjustment or reevaluation of the regimen.

Do not use Testosterone Cypionate injections if you have or might have prostate cancer or breast cancer. You also should not use this product if you are allergic to any part of the medicine, including the oil (sesame oil).

Providers must use caution with patients who have serious heart, liver, or kidney problems, as this medicine can cause the body to hold onto fluid. If you have sleep apnea or a history of blood clots, inform your doctor before use.

Compounded testosterone injections like Testosterone Cypionate can affect how well other medications work, so patients should provide their doctor with a list of medications they take. This product can make blood thinning medications stronger, meaning the dose of the blood thinner might need to be lowered.

Testosterone may change blood sugar levels in patients with diabetes, requiring possible adjustments to their diabetes medicine. Talk to your doctor about all new medications or supplements.

Compounded Testosterone Cypionate injection vials should be stored under refrigerated conditions. Before using, look at the medicine. It should be clear or light yellow with no particulates floating in it.

How to Administer a Compounded Testosterone Injection

A Testosterone Cypionate injection is an intramuscular injection (also referred to as an IM injection) and can be injected subcutaneously as well.

For IM injections, the product should be injected into large muscle groups such as the vastus lateralis muscle of the thigh or the gluteal muscle of the buttocks. Standard disposable syringes with a longer needle length should be used to administer the injection deep within the muscle tissue.

To inject Testosterone Cypionate subcutaneously, use a thin needle (such as a 25G to 27G, 5/8-inch needle) to target the fatty tissue layer of the abdomen or outer thigh. Gently pinch a fold of skin and insert the needle at a 45-degree angle. Because the medication is oil-based, depress the plunger slowly and steadily over 10 to 15 seconds to ensure comfortable delivery into the fat layer.

Before you inject a prescribed dose, be sure to thoroughly clean and sterilize the injection site. An alcohol wipe is recommended, but warm water and soap can also be used to accomplish this step.

When it is time to apply the injection, consult your doctor to see if you should inject subcutaneously or intramuscular. For intramuscular, angle the needle at 90-degrees to reach the muscle. For subcutaneous, angle the needle 45-degrees. Keep in mind that different needles may be used based on the method of application. To prevent localized tissue damage and irritation, it is strongly recommended to rotate the injection sites with each dose.

Consult your medical provider or nurse to learn the proper technique for administering a subcutaneous or an IM injection safely. Have additional questions? Contact our compounding pharmacy.

Frequently Asked Questions

Andropause is a term used to describe the age-related symptoms associated with decline in male hormones, primarily testosterone. Symptoms of andropause look differently for all men, but the most common include fatigue, loss of muscle mass, low libido, erectile dysfunction, mood swings, difficulty sleeping, and weight fluctuations.

Low T refers to insufficient levels of testosterone, the primary male hormone known for maintaining muscle mass, bone density, energy, and sexual health. It is often seen as the primary cause of andropause symptoms. In men, low testosterone levels are directly linked to decreased sperm production and fertility.
